Comparative study between wise pattern and non-vertical scar reduction mammoplasty

Abstract

Breast reduction is one of the common procedures that is done every day. Indications for reduction include many aspects like aesthetic, functional and therapeutic. Therapeutic breast reduction for management of breast cancer adds functional and aesthetic results for many ladies. Wound healing time is important factor to receive adjuvant treatment. Many techniques were used for reduction. Wise pattern is one of the commonest methods for reduction. Other technique like non-vertical scar offers fewer wounds and fewer scars.  

Aim: of study is to compare between wise pattern (WP) and non-vertical (Horizontal) scar regarding result satisfaction, wound healing time and complications with focusing on wound complications.

Patients and methods: 106 patients were operated between June 2019 and August 2022. First group (WP): 38 patients were operated by wise pattern technique with superior or inferior pedicle while second group (H): 68 patients were operated by nonvertical scar reduction using the inferior pedicle technique. Breast Q module 1 for reduction was done at 1 year post-operative. To evaluate satisfaction: module of breast and module of nipple calculated separately to measure satisfaction for each item. 

Results: Wound complications occurred in 50% of cases for WP group and needed mean 32.5 ±12.72 days for complete healing. While for H group wound complications occurred in 38.23% of cases with mean 25.4 ±6.8 days for complete healing with P value 0.03 denoting significant difference. Also Breast Q for nipple showed significant difference with P value 0.000 as it was 73.42% ±7.82 for WP and 86.74% ±7.43 for H group. Other parameters didn’t show significant difference 
  
Conclusion: Horizontal breast reduction is a safe method for reduction with considerable satisfaction rate.  And less wound healing time than WP reduction.

Dr Ahmed Elghazaly is a consultant breast surgeon and dedicated his career for breast surgery…. He completed MD degree in 2018. He also had IMRCS of England in 2009 and master degree in 2009. He is working in breast unit in Tanta Oncology center which is a high volume oncology Center.
