Problems of lymphorrhea and their overcoming after surgical interventions in oncogynecology

Abstract

Lymph node dissection is one of the key points of surgical interventions in oncogynecology. Phenomena of lymphorrhea of various degrees of severity occur in the vast majority of cases after treatment with removal of lymph nodes. The following types of pathological leakage from the lymphatic channel are distinguished: lymphatic ascites, lymphocele, lymphorrhea, lymphatic fistula, as well as special forms of lymphatic leakage: chylous ascites, chylorrhea, chylothorax, etc. The following complications of lymph flow are important in the clinic: dehydration, nutritional deficiency, immunological dysfunction, increased risk of infection, compression of vital structures in case of chylous ascites, lymphocele, and chylothorax. Diagnosis of postoperative lymphorrhea includes assessment of clinical symptoms, as well as additional research methods such as oral test with contrast, CT, lymphangiography, lymphoscintigraphy, laboratory examination of drainage contents and diagnostic paracentesis. There are conservative and surgical approaches in the treatment of postoperative lymphorrhea. Thus, primary, secondary, and tertiary prevention of lymphorrhea should include a set of measures in preoperative preparation, during surgery, and in the postoperative period, such as control of chronic diseases and improvement of protein status, minimization of tissue trauma and wound drainage, control of the lymphorrhea volume, and control of water-electrolyte balance. Effective prevention of lymphorrhea requires a multidisciplinary approach with the participation of surgeons, oncologists and rehabilitation specialists.

Biography

Iegorov Mykhailo Iurievych, born on January 26, 1980, in Kyiv, Ukraine, is a distinguished medical professional specializing in obstetrics, gynecology, and oncogynecology. He graduated from the National Medical University named after O.O. Bogomolets in 2006, specializing in Primary Care, which encompasses General Practice, Family Practice, and Internal Medicine. He completed his internship in obstetrics and gynecology at the National Medical University named after Danylo Halytsky in Lviv (2006-2009) and pursued clinical residency at the National Medical Academy of Postgraduate Education named after P.L. Shupyk in Kyiv (2009-2011). In 2013, he advanced his expertise by completing specialization courses in oncogynecology at the National Cancer Institute, Kyiv. Further enhancing his qualifications, he completed a correspondence postgraduate course in obstetrics and gynecology at the National Medical Academy of Postgraduate Education in 2016. In 2017, he defended his dissertation on "Criteria for Differential Diagnosis and Rationale for Pathogenetic Treatment of Patients with Benign and Borderline Epithelial Ovarian Tumors." Since 2018, Dr. Iegorov has been serving in the Scientific and Practical Department of Female Reproductive Neoplasms at the National Cancer Institute. He is a prolific author and co-author of over 10 scientific articles in gynecology and oncogynecology, contributing significantly to his field.
